  • A Social Network connecting with friends/others in your community by coming together to care for each other.

  • Community Care as a web of relationships in which we support each other to build trust, connection, and improve community wellbeing.

How can I offer/request support?

Examples:

  • Purchase an internet subscription for your neighbor so their kids can access more educational activities.

  • Send supportive text messages to a lonely older adult.

  • Show an isolated elderly neighbor how to use Zoom/Facetime so they can stay connected with family.

  • Support a neighbor with practicing their speech exercises on Facetime as they recover from a Traumatic Brain Injury.

  • A home page called ‘Bright Spots’: posts of moments/stories of bright moments that made you and your neighbors smile!

  • These could be moments of offering/receiving support, or just bright moments from your day-day life. We need to be reminded that there are bright moments happening all around us all the time!

My Support Network

  • Invite friends/neighbors and others to be a part of your ‘Support Network’: individuals that you can call on when you need support.

  • Asking for help/support is hard. It’s easier if you already have a list of friends and people that you have asked to be in your support network and will answer your call when you need it. The hardest part of asking for help is already over.

Community Care Teams

  • When offering or receiving support, everyone joins a Community Care Team: a group of community members that wrap around and support an individual or family.

  • Join with your friends, family, and partners! This is a great way to spend time and connect with people you already know and trust.
